Senior Care Coordination

Harmonia Senior Care Managers offer compassionate care to seniors facing the challenges of lifestyle change, committed to helping you live the best quality of life possible with dignity and respect. Our goal is to help you remain independent in your home. Our service areas include Springville, Hamburg, Evans, Eden, North Collins, Collins and Gowanda.

Senior Services are funded in part by the Erie County Department of Senior Services, the NYS Office of the Aging, and the Administration on Aging. Some programs we refer you to require payment for services; our financial assessment test will determine ability to pay. Private pay options are also available.

Information, Referral and Assistance

Let us provide you with: • Lists to housing and assisted living options in Erie County • Lists and resources for area service providers

Entitlements

We can assist with applications for HEAP, SNAP and Meals on Wheels.

Community Linkage

We can help link you to area vocational or social programs, as well as support groups for caregivers.

Linkage to Geropsychiatric Services

This service can be obtained through referral to our counseling services and includes: • Geropsychiatric Assessment • Psychiatric Consultation • In-home Psychotherapy • Collaboration with Physicians • Individual and Family Counseling

What You Can Expect

When you call for information or services you will connect with the coordinator of the Department. A general list of questions will be asked.

If needed, you will then be assigned to a Case Manager for further assistance. The Case Manager that will be assigned to your case will dig deeper and help walk you through the complex system. 

Our goal is to help you remain independent in your home. We like to consider ourselves as the missing link between the client and the service. An in-depth state assessment will be completed if your case manager determines the need. Often times people will ask "Why all of these questions just for Meals on Wheels?". Although it takes some time, these questions will help us develop a personalized care plan that is best suited for your needs.

Trained Case Managers will help review other programs and benefits that may be best suited for you. Once you are connected to a specific service, you will be monitored by phone or in person at your home.
