Harmonia’s trained CarePanions offer personalized, in-home support to seniors or other individuals in need. CarePanions help you or a loved-one remain independent and safe at home with punctuality, empathy and hardwork. A personal care plan is developed for each participant, matched to meet your specific needs and interests.

Case Managers will assess your eligibility for county funding. Private pay options are available.

Homemaking

Allow us to provide light house-keeping services and assist with food preparation.

Monitoring Safety of Home Environment

Our trained CarePanions keep an eye on you or your loved one, identifying tripping hazards or areas of concern.

Errands and Grocery Shopping

Let us be your “personal assistant”. We can help you do the necessary things that help you or your loved one maintain their independence.

Companionship

Whether engaged in an active task, or passively spending time with you or those you love, our CarePanions provide that “relationship” piece which is so important in healthy aging.

What You Can Expect

Private Pay

You are considered for services when you are referred to us by either yourself, a family member or care provider. Our CarePanion Coordinator will contact you and discuss the program. Then an Intake will be scheduled for our Coordinator to meet with you and review the paperwork to determine what your needs.  

Next you will be assigned to one of the CarePanions that will best fit your needs. The CarePanion will be in contact with you to schedule your start of service.

The CarePanion Coordinator will follow-up periodically to conduct home-visits to ensure there are no issues with the services being offered. If at any time there are any issues or problems, you can reach out to the Carepanion Coordinator to have it addressed as swiftly as possible. 

County Funded

Your consideration for services begins when our agency sees your case on the county home care list; if you are within our service area, we request to provide services to you through the Erie County Department of Senior Services. 

After the agency has the approval to serve you, the Coordinator is in contact with you and your Case Manager to notify them that you will be receiving services through our agency.   

Next a CarePanion will call you to schedule a day and time to meet with you and start your services. 

In the meantime, you will receive a ”check-list” in the mail that we would like for you to complete and leave on your fridge or kitchen table so your CarePanion, Case Manager or family can see what is being done at each visit to help you.
