Counseling Services
Our professionally licensed clinicians provide individual, couples, family, and group therapy for individuals aged 10 and older. Using a holistic, empathic framework, we tailor each treatment plan to the whole person, not just the presenting diagnosis.
Counseling services are covered by most health insurance plans, including Medicaid, Medicare, and most private insurers. Co-pays and co-insurance are due at the time of service per your insurance agreement.
What To Expect
Starting therapy is a simple process. When you reach out, our front desk team will gather some initial information to help match you with the right clinician.
During your first one to three sessions, your therapist will conduct a thorough assessment to understand what’s bringing you in and what your goals are. From there, you’ll collaborate on a personalized treatment plan.
Wondering if your therapist is the right fit? We’re here to help. Therapeutic alliance is one of the strongest predictors of positive outcomes, and we’ll work with you to find the right match. Keep in mind: a good therapist will challenge you. That’s part of the process.
As you grow and progress, the frequency of sessions typically decreases. The goal is always your independence, not dependence on us.

Senior Care Coordination
Harmonia’s Senior Care Managers provide compassionate, person-centered support to older adults navigating the challenges that come with aging. Our goal is to help you or your loved one maintain independence, dignity, and quality of life at home.
Services are available in Springville, Hamburg, Evans, Eden, North Collins, Collins, and Gowanda. Funding is available through the Erie County Department of Senior Services, the NYS Office for the Aging, and the Administration on Aging. Private pay options are also available; a financial assessment determines eligibility for subsidized services.

What to Expect
Your first contact with our team connects you with our department coordinator, who will gather general information and, if needed, assign you to a dedicated Case Manager. Your Case Manager will complete a comprehensive state-level assessment to build a personalized care plan that addresses all of your needs, not just the immediate ones.
Once connected to services, you’ll receive ongoing monitoring by phone or in-person home visits to make sure everything is working for you.
Support for Caregivers
We see you, and we appreciate you. Harmonia offers caregiver-focused educational events in partnership with the Alzheimer’s Association and Hospice. Have a specific need or question? Give us a call – we’re here to support the people who support others.
Health Home Care Coordination
Harmonia’s Health Home Care Coordinators help Medicaid-eligible adults with complex health needs stay healthy, stable, and out of the emergency room. We coordinate your care across providers and connect you to community-based resources at no cost to you.
Who Qualifies
Health Home services are available to adults aged 21 and older enrolled in Medicaid who meet one of the following criteria:
- Two or more chronic health conditions (such as asthma, diabetes, heart disease, or a mental health or substance use disorder)
- A single qualifying condition: HIV/AIDS or Serious Mental Illness (SMI)
At least one of the following risk factors must also be present: homelessness or risk of homelessness, lack of social support, non-adherence to treatment, learning or cognitive challenges, or deficits in daily functioning.

What to Expect
Your care coordinator will serve as your advocate and guide through the healthcare system. Services are person-centered, starting with a care plan built around your goals, values, and needs. Your coordinator will:
- Connect you to medical doctors, dentists, specialists, and mental health or substance use programs
- Ensure your providers are communicating with each other, so you don’t have to manage it alone
- Help arrange transportation to medical appointments
Assist with benefits applications (SNAP, Cash Assistance, HEAP, weatherization, and more) - Support educational and employment goals, including GED programs and financial aid
Our care coordinators serve all of Erie County and can meet you at home or wherever you’re most comfortable. All services are provided at no cost to enrolled participants.
